Completing this form will help us determine if any of our facilities are suited to your needs and are available on the dates and times you request. FACILITIES ARE NOT CONFIRMED until you receive formal agreement from our Facilities Department. Additional information may be required before that time.

  • Certificate of Insurance (Naming Bethany as Additional Insured)
  • 501 (c) (3) Determination Letter
  • Organization’s Mission Statement
  • Brochures describing your organization
